This will be a review of all incidents and accidents that have happened in Northwest Ohio, at or near Toledo Express, or where Toledo Express was the origin or destination. Information is provided primarily by ASN and NTSB archives.
Fatal Accidents
- January 12, 2008 - Port Clinton, OH - N2647Y Cessna 340: 4 Fatal Injuries NTSB Report
- September 26, 2007 - Defiance, OH - N8341G Piper PA-32R-301T: 1 Fatal Injury NTSB Report
- September 3, 2007 - Kelley's Island, OH - N1301Y Cessna 172C: 2 Fatal, 1 Serious Injury NTSB Report
- November 30, 2004 - St. Louis, MO - Grand Aire Express MBB HFB-320 Hansa Jet : Service flight, operated by N604GA, returning the aircraft to Toledo from Spirit of St. Louis Airport in St. Louis, MO crashed on takeoff. The cause of the accident was attributed to a failure of proper maintenance on the elevator trim for the aircraft and pilot error is maintain proper clearance from terrain. Report at ASN
- April 8, 2003 - Toledo Express - Grand Aire Express Dassault Falcon 20 : Cargo flight, operated by N183GA, returning to Toledo from Traverse City, MI crashed in Oak Openings Metropark west of the airport on a go around after an aborted takeoff. The crash was attributed to crew error and icing conditions being experienced at the time. All three occupants, including 2 crew members, perished in the crash. Report at ASN
- February 15, 1992 - Toledo Express - BAX Global DC-8 : Flight 805, operated by N794AL, inbound from Seattle was unable to properly capture the ILS localizer for approach during foggy conditions. After two failed approaches, the aircraft made impact with the ground north of airport. All four occupants, 3 crew and 1 passenger, died. Report at ASN, N794AL Photos at Airliners.net
- August 2, 1986 - Toledo Express - American Agronomics Corp Hawker Siddeley HS-125-1A-522: Aircraft, N50HH, departed Toledo Express for Bedford, IN and experienced no issues in flight. On touch down at Bedford, the aircraft landed two-thirds of the way down the runway and a go around attempt failed resulting in he aircraft overrunning the runway and catching fire. The two crew members were killed in the crash. Report at ASN
- October 29, 1960 - Toledo Express - Arctic Pacific Curtiss C-46F-1-CU Commando - : Charter flight, operated by N1244N, for the California State Polytechnic College football team departed Toledo Express for San Luis Obispo, CA crashed on takeoff. Probable cause was noted as aircraft being overweight, weather conditions, and partial loss of power in the left engine all resulting in loss of control during a premature lift-off. A total of 22 of the 48 occupants, including 2 of the 3 crew members, did not survive the crash. Report on ASN

- February 1, 2004 - Pittsburgh, PA - Shuttle America dba US Airways Express N158SD [{Saab 340]]A: Flight 4593 destined for Toledo Express was struck and damaged by a baggage cart at Pittsburgh's Gate E11. NTSB Report

Emergency Landings
- United Airlines Boeing 727-200 Sept 23, 2000: A United Airlines Boeing 727-200 made an emergency landing after being struck by lightning. It was flight 678 operating from Chicago O'Hare to New York.
